DLSE Excel Spreadsheet and google form

I am a labor rights attorney working in California. I am trying to get an online form that will ask the questions of my clients I need to fill in a DLSE initial wage claim report form. I want the spreadsheet to have an online component (so it can be done remotely) and have a macro that can automatically fill the DLSE pdf. I eventually want that in english and Spanish. I am attaching the form.
